To the Chairman and Members of the Vestry of St. Mary, Islington. Gentlemen, POPULATION. The statistics contained in this report are calculated on an estimated population of 331,901 persons, of whom 156,326 are males and 175,575 are females. The following Table gives the estimated population at nine age periods. Table I. There were 5,388 persons married during the year, so that there were 16.23 persons married for every 1,000 persons living in the Parish. In 1893 the rate was 16.16, and in 1892, 17.14. The marriages and the persons married rate during the four quarters were as follows:— Period. Marriages. Persons married. Persons married, rate per 1,000 population. 1893. Marriages. There were 9,502 births registered, of which 4,862 were males, and 4,640 females. These were equal to an annual birth rate of 28.62 per 1,000 of the population, which is the lowest rate recorded since 1871. A glance at column three in Table VI. shows that ever since that year the birth rate has been steadily declining. The proportion of females born to every 100 males was 95.4, which is a slight increase on the proportion of 1893. The excess of births over deaths, or the natural increase of the population, was 4,239. This was a better return than any since 1889, and was entirely due to the very low death rate of the Parish. There were 5,263 deaths registered during the year, compared with 6,891 in the preceding year, so that the returns showed a decrease of 1,128. These deaths were equal to an annual death-rate of 15.85 per 1,000 inhabitants, which is the lowest annual death-rate that I can find any record of in the Parish. In this connection it may be noted that the death-rate of Islington since 1851 has been, for decennial periods, as follows :— 1841-50 19.0 per 1,000 inhabitants. 1851-60 21.0 „ „ „ 1861-70 22.0 „ „ „ 1871-80 21.9 „ „ „ 1881-90 18.5 „ „ „ Thus it will be seen that there has been a steady improvement in the death-rate since 1861, and this, too, despite the fact that the Parish has, decade by decade, steadily become more and more crowded. At the present time Islington is more than four times more densely populated than it was in 1840. I have calculated that In 1841-50 there were 0.041 acres to every inhabitant „ 1851-60 „ 0.025 „ „ „ „ 1861-70 „ 0.017 „ „ „ „ 1871-80 „ 0.013 „ „ „ „ 1881-90 „ 0.010 „ „ „ Yet, in spite of this ever increasing aggregation of people, it says a great deal for sanitation that it has not only prevented the death-rate from becoming higher, but has actually reduced it, so that in 1894 it was as low as 15.85, 11 The nearest approach to this rate was in 1889, when, with 5,093 deaths, it was 16.3, and in 1888, when 5,206 deaths gave 16.9. The Islington death-rate compares very favourably with that of the Metropolis, in which it was 17.4 per 1,000. From a return in The Lancet I have been able to obtain statistics relating to the neighbouring districts :— Deaths. Death-rate. St. Marylebone 2,662 = 19.3 Hampstead 798 = 10.6 St. Pancras 4,256 = 18.3 Islington 5,263 = 15.8 Stoke Newington 334 = 9.8 Hackney 3,241 = 15.4 Clerkenwell 1,243 = 19.1 St. Luke 965 = 23.5 When I turn to the mortality of the large towns I find that only five out of thirty-three populous places had a mortality rate as low as that of this Parish. These were Croydon (13.2), Portsmouth (15.2), Leicester (14.7), Derby (15.0), and Huddersfield (15.8). The populations of these places are not, however, nearly so great as that of Islington, the largest—that of Leicester—being 189,136. When the death-rate of Islington is contrasted with those of the greatest communities it is then seen in a particularly favourable light:— Population. Deaths. Death-rate. Birmingham 492,301 9,125 18.6 Liverpool 507,230 12,062 23.8 Manchester 520,211 10,592 20.4 Leeds 388,761 6,927 17.9 Sheffield 338,316 5,994 17.8 Islington 331,901 5,263 15.8 12 MORTALITY IN THE SUB-REGISTRATION DISTRICTS. The mortality in the sub-districts was equally satisfactory, and in two of them especially so. Thus, in Upper Holloway only 1,458 persons died, producing a death-rate as low as 14.86. In Highbury the mortality was even proportionally less, for, with 865 deaths, the death-rate was only 14.01. In Islington South-west, which is the most populous as well as a very densely crowded Sub-district, 1,859 deaths were registered, and the death-rate was 17.34. In Islington South-east, which is the most densely crowded Subdistrict, the deaths numbered 1,081, which were equal to an annual death-rate of 16.64. Considering the density, as well as the character of the population, of these two last-mentioned Sub-districts I am of opinion that their death-rates are in the highest sense of the word satisfactory. The following Table shows the number of persons, males and females, who died in each Sub-district, and also the several deathrates of such Sub-districts : — Table VII. Registration Sub-Districts. The deaths registered from the seven principal zymotic diseases amounted to 796. These are 35 below the mean number (831 for the preceding ten years after correction for the increase of population. In the previous year there had been 871 deaths, so that there was a decrease of 75 on that return. The death rates for these three periods were:— 1894 2.39 per 1,000 inhabitants. 1893 2.65 „ „ 1884-93 2.50 „ „ The death rate for the year compares with that of London and the 33 great towns, thus :— Islington 2.39 per 1,000 inhabitants. London 2.63 „ „ 33 Large Towns 2.43 „ „ 32 Provincial Towns 2.26 „ „ Compared with the neighbouring districts, the results were also satisfactory, for although the death rate was higher than that of Hampstead, Stoke Newington, St. Luke, Marylebone and St. Pancras it was lower than that of Hackney, Clerkenwell and Shoreditch The death rates in these places were as follows:— Islington 2.39 per 1,000 inhabitants. Marylebone 2.36 „ ,, Hampstead 1..37 „ ,, St. Pancras 2.18 „ ,, Stoke Newington 0.84 „ ,, Hackney 2.61 „ ,, Clerkenwell 2.66 „ ,, St. Luke 2.11 „ ,, Shoreditch 2.84 „ ,, The principal factors in the Zymotic death-rate of the Parish were Measles, Diphtheria and Whooping Cough, each of which were in excess of the returns of the preceding year. Measles was excessive in South-west Islington where there were 106 deaths, which were equal to a death-rate of 0.98. Diphtheria was most fatal in Upper Holloway, where 70 deaths produced a death-rate of 0.71 per 1,000 of the population. Whooping Cough produced its greatest mortality in South-west Islington, where it carried off 79 children, which gave a death-rate of 0.73. 16 In the annexed Table a complete statement of the deaths from the Infectious Diseases during the years 1881-93 and in 1894 is given :— Table XI. During the year very full reports were published on the health of the district at the close of each quarter, so that it will not be necessary now to add much under this head. The Zymotic Diseases were most fatal in the Second Quarter of the year, chiefly owing to the large number of deaths from Measles, from which disease 118 fatal cases were known, producing a death rate of 1.42 per 1,000 inhabitants. In the Fourth Quarter the deaths from the Zymotic Diseases only numbered 105, while in the First, Second and Third Quarter they were respectively 231, 277 and 183. Table XIV. Cancer showed its greatest mortality in the fourth quarter, when it carried off 79 persons, compared with 45 in the first, 50 in the second and 65 in the third. Local Diseases were most in evidence in the first quarter, when they were responsible for 822 deaths, and least so in the third quarter when there were only 467. In the second quarter there were 559 deaths, and in the fourth 606. In the sub-division of Local Diseases the deaths arising from the Nervous System were most fatal in the first quarter, when 157 persons died, and least so in the third quarter, when 123 persons succumbed. In the second and fourth quarters the record of deaths was the same, namely, 133. Diseases of the Circulatory System were practically similarly fatal in the 1st and 4th quarters, when 105 and 103 persons died. In the 2nd and 3rd quarters the deaths numbered 89 and 75. Diseases of the Respiratory System were far and away more fatal in the Ist quarter than in any other period of the year, for then the 26 deaths of 402 persons were registered. In the 4th quarter of the year 246 deaths were known, and in the 2nd and 3rd quarters the numbers were respectively 202 and 123. Diseases of the Digestive System were most numerous in the 3rd quarter, when 92 deaths were registered. In the 4th quarter 82, in the 1st quarter 83, and in the second quarter 73 persons died. The number of notified cases, 3,120, showed a very satisfactory decrease on that reported in the preceding year, when 4,851 cases were known. It is also less than the return of 1892 when 3,309 certificates were received. The figures for each of the years since the notification of Infectious Diseases became compulsory, are as follows:— 1891 2,014 1892 3,309 1893 4,851 1894 3,120 It is a most notable fact that the decrease was not confined to one disease alone, but was common to each of those that are notifiable. Thus Small Pox showed a decrease of 29 cases, Scarlet Fever of 1,386, Diphtheria and Membranous Croup of 17, Enteric and Continued Fever of 6, Erysipelas of 277 and Puerperal Fever of 15. These make a total decrease on the preceding year's returns of 1,731 cases, which is eminently satisfactory. The principal decline occurred with Scarlet Fever, which fell from 2,879 eases in 1893 to 1,493, or a decrease of 51.8 per cent. In the various wards of the parish the number of notifications for 1893 and 1894, were as follows:— 1893 1894 Decrease. TRADES AND OCCUPATIONS AND INFECTIOUS DISEASES. It is sometimes suggested that the notification of infectious diseases is of very little real service, and that it is a mere waste of money to pay for it. I think, however, that the subsequent tables (XXXVII. and XXXVIII.), which I have compiled, amply disprove these assertions. In the first table are given the trades or occupations of the persons attacked, and in the second the trade or business carried on in the premises in which the disease occurred. It will be seen from them that many of the businesses therein mentioned are such as would easily lend themselves to the propagation of disease, if precautions were not at once adopted to prevent such an occurrence. In every instance this was done, either by the removal to hospital of the patient, or, where this was not agreed to, by the cessation of the business. The results have been all that could be desired, so that in no case was the disease known to spread to others by reason of the business. In my quarterly reports, I have given at considerable length particulars of the Sanitary work which has been carried out by the Public Health Department. As this matter has been so fully laid before you, I do not propose to go over all the ground again. I may, however, say that in no previous year in the history of the parish has so much been undertaken and accomplished. INTIMATION NOTICES. At the beginning of the year a new form of Preliminary or Intimation Notice for the abatement of nuisances was put by me, before the Public Health Committee for their consideration, and adopted by them for future use. Prior to this time, the Preliminary and the Statutory Notices were precisely the same in form, and consequently the public were unable to readily distinguish between the two. The following is a copy of the Notice which was adopted. Form A. THE VESTRY OF SAINT MARY, ISLINGTON. PUBLIC HEALTH (London) ACT 1891. INTIMATION. Public Health Department, Vestry Hall, Upper Street, N., 189 To the OWNER, Sir or Madam, The Sanitary Inspector, having visited the above premises, has given me information that the nuisances numbered in the schedule at the back hereof, which are liable to be dealt with summarily, exist thereon. I, therefore, now by this written intimation, make the existence of the said nuisances known to you, as being the person who is required to abate them. And I have to request that the same be abated within the period of days 71 At the end of this time the Sanitary Inspector will again visit the premises, and if the necessary works have not then been completed, the Vestry, as the Sanitary Authority for the Parish, will commence proceedings against you by the service of a Statutory Notice.* I am, Your obedient Servant, Medical Officer of Health. * Section II (I) of the Public Health (London) Act, 1891, enacts as follows:—"All reasonable costs and expenses incurred in serving notice, making a complaint, or obtaining a nuisance order, or in carrying the order into effect, shall be deemed to be money paid for the use and at the request of the person on whom the order is made; or if no order is made, but the nuisance is proved to have existed when the notice was served or the complaint made, then of the person by whose act, default, or sufference the nuisance was caused; and in case of nuisances caused by the act or default of the owner of premises such costs and expenses may be recovered from any person who is for the time being owner of such premises." If it become necessary through your default to serve a Statutory Notice, the cost incurred will be three shillings, which will be recovered as provided above. All communications respecting this Notice to bo addressed "The Medical Officer of Health, Vestry Hall, Upper Street, N." Attention is directed to Section 42 of the Public Health (London) Act, 1891 "If a water-closet or drain is so constructed or repaired as to be a nuisance or injurious or dangerous to health, the person who undertook or executed such construction 73 or repair shall, unless he shows that such construction or repair was not due to the wilful act, neglect, or default, be liable to a fine not exceeding twenty pounds." No drainage or other works are to be covered in until inspected and approved by an Officer of the Vestry. Before commencing any Drainage Works it will be necessary to give notice of your intention, and to submit plans at this Office. All works must be carried out in conformity with the London County Council By-Laws and the Regulations of this Vestry. The schedule of nuisances on the back of the notice has been the means of saving a great deal of clerical labour, and it has also proved itself a useful aid in sanitary administration. Up to the month of January it had been the custom of the Inspectors to make out both the originals and copies of the preliminary notices, but from that date this duty has devolved on a clerk specially appointed for the purpose. The practice now adopted is for the Inspector, who requires the preliminary notice, to fill up a requisition form on which he writes down the numbers of the nuisances as described in the schedule. The requisition is then given to me and carefully examined before it is passed on to the clerk who writes out the notice. If it contained any demand which I considered of an exceptional or unnecessary character I made it my duty to consult with the Sanitary Inspector so as to ascertain the reason for his requirement, and if it then appeared to me necessary or reasonable it was allowed to remain on the notice, but if not, it was struck out. OUTWORKERS. A commencement was made in the early part of the year to ascertain the names and addresses of the Outworkers employed in Islington, and for this purpose Inspector Pain was taken off his district. He was engaged on this work up to the time of his death in July. Prior to his demise the loss of his services from his district caused a great deal of confusion in the Sanitary Department, as owing to the large amount of work that was going on it was almost impossible to spare him. 74 The Public Health Committee, however, felt that an attempt should be made to meet the requirements of the Home Office with respect to these persons, and accordingly Inspector Pain was selected for the work. His first duty was to visit the establishments which employed Outworkers, but this proved so enormous that practically he was unable to do little more than ascertain from the employers the addresses of the people engaged by them. It frequently happened that when an employer gave him the name and address of an Outworker, and that when the Inspector called at the address he discovered that the person designated as an Outworker by the original employer, was himself also the employer of several other hands, who executed their work at their several homes. It was also frequently found that after a short interval on his again calling on the employer, that the names of the persons on the list on the previous occasions had been struck off, and others put on in their stead. It was also often discovered that many of the hands had not only ceased their employment for the particular tradesman, but were also unemployed by any other persons. This must naturally be so, because there are certain trades which will only be busy at certain periods of the year, while at the other seasons they are practically idle. Prior to Inspector Pain's death I recommended that an Inspector should be specially appointed for the purpose of looking after workshops, workrooms, and the places where people are employed. At the end of the year this matter was engaging the serious attention of the Public Health Committee. DISINFECTING STATION. If the year were remarkable for no other work it would be remembered because of the erection of a Disinfecting Station for the use of the Parish. The want of such a sanitary defence had been felt very acutely for a long time, for it was anything but satisfactory that so important a 75 work as that of disinfecting large quantities of clothes and articles taken from houses in which infectious disease had broken out, should be performed by contractors, over whom the Vestry had little or no control, and in a machine that by no means met the requirements of modern sanitation. The process employed by the contractors was that of disinfection by means of hot air. Since it was found possible to utilise steam under pressure for the purposes of disinfection it has been generally adopted, as being the best means whereby thorough disinfection can be assured. When the Public Health Committee entertained the idea of erecting Disinfecting Chambers they unanimously came to the conclusion, after viewing the machines of several makers, to adopt those made by Messrs. Goddard, Massey & Warner, of Nottingham. In the scheme that was adopted for the Disinfecting Station provision was made for the erection of two of that firm's steam disinfectors. It may be that for ordinary purposes the work could be done by a single apparatus, but it was felt that at times, as during the recent epidemic of Scarlet Fever, two machines would be required to disinfect the large quantities of bedding and clothing that had been infected.* It was also felt that with one disinfector if any break-down should occur, that then the Parish work would be greatly incommoded by having again to resort to a private contractor, with all its attendant difficulties and inconveniences. Inasmuch as the cost of an extra machine was only a matter of £240, without the buildings, the Public Health Committee recommended the erection of the second disinfector without hesitation, and I am glad to say the Vestry unanimously adopted their suggestion. The Disinfecting Station consists of two disinfecting chambers, a laundry, an office, two shelters for the vans which are to convey the infected and disinfected clothes respectively, and a crematory for destroying infected bedding, &c. The addition of a laundry to a Disinfecting Station, if not absolutely a new departure, is certainly one that has hitherto not * Since the above paragraph was written it has been found necessary on several occasions to use the two disinfectors. 76 been generally adopted elsewhere. I felt no difficulty in advising the Vestry to erect this portion of the Disinfecting Station, because in the past when I had been Medical Officer of Health to the County Borough of Sunderland I had very frequently seen articles, bedding especially, brought for disinfection, in a state of filth that was very obnoxious. And I was also aware that in several instances after they had been returned home and when fresh disease had broken out in the house their uncleanliness had been put down as the cause of the fresh outbreak, although, as a matter of fact, having been thoroughly disinfected by steam, they were quite free from infection. In a modern steam disinfecting apparatus, properly managed, such articles could not have contained any living germs. It was, however, most unsatisfactory from several points of view, that such an impression should have prevailed. In Islington, in the future, clothing, bedding and other articles, that have been soiled, will not be returned to the owners' homes before they have been thoroughly cleansed at the hands of a careful laundress. I believe that this Station, when it is shortly opened,* will be found to be the best of its kind, not only in London, but in the United Kingdom, and I foresee it will prove a very great boon to the community and for ever prevent any suspicion being attached to clothing which had been disinfected, as being the cause of a new outbreak. PUBLIC DISINFECTION. Street Watering and Disinfectants.—The mixing of Carbolic Acid with the water used in street watering was continued during the summer months, with satisfaction to the Public Health Department. Courts, Lanes, and Street Channels.—Carbolic Acid Disinfecting Powder was applied during the hottest portions of the year to these places, and also to the channels in some of the dirtiest parts of the district. * The Station is now open, and doing a very large amount of work at a decreased cost to the ratepayers. 77 Dust-bins.—These were also disinfected (after emptying by the Dustmen) during the long period that the weather continued warm. This building is now completed and will shortly be ready for occupation. It has been erected in compliance with Sec. 60 (4) of the Public Health (London) Act, 1891, which declares that "The Sanitary Authority shall provide, free of charge, temporary shelter house accommodation with any necessary attendants for the members of any family in which any dangerous infectious disease has appeared, who have been compelled to leave their dwellings, for the purpose of enabling such dwellings to be disinfected by the Sanitary Authority." The building consists of two separate bindings, one of which is a caretaker's dwelling, while the other consists of four sets of rooms which are provided for the temporary shelter of persons who have been removed from their homes during the period of their disinfection by the Vestry. Each set of rooms is complete in itself, and has no communication with any other portion of the building. There is thus provision for four families, to each of whom is assigned three rooms, namely, a living room, a bedroom, and a bathroom, which is supplied with hot and cold water. Each set of rooms is also provided with a water closet. The building was designed by Mr. J. Douglas Matthews These buildings will afford a long felt want in the district, for it has frequently occurred that persons have had to be provided with lodgings under circumstances such as they are intended to meet. They are situated at the rear of the Hornsey Road Baths, and are not far removed from the disinfecting station. Entrance is obtained by means of a private road on the north-west side of Seven Sisters Road. SANITARY INSPECTORS' WORK. During the 12 months 24,417 separate and distinct nuisances, as mentioned in the schedule, were dealt with by the Sanitary Inspectors, of this number 5,605 came under observation in the 1st Quarter, 6,182 in the 2nd Quarter, 6,955 in the 3rd Quarter and 5,676 in the 4th Quarter. 79 In addition to these nuisances, there were about 2,000 nuisances dealt with which were not included in the schedule of nuisances already referred to. This is an enormous record of work, and is far and away greater than anything that has been attempted in the Parish in past years. COWHOUSES. These places were duly inspected at intervals by the Sanitary Staff. They were also visited by the Public Health Committee, who directed me to oppose the granting of the licenses of two Cowhouses situated at 129, Newington Green Road, and in Mildmay Avenue. This was done, with the result that they were struck off the register at the Annual Licensing Sessions of the London County Council, held at Spring Gardens. SLAUGHTER-HOUSES. The Slaughter-houses were also visited by the Sanitary Inspectors on 86 occasions, as well as by the Superintendent and myself. Generally speaking they were in a far better condition than I had known them to be in previously. It was my duty, however, to object to the renewal of the licenses of the Slaughter-houses situated at 2, Durham Road and 13, Brandon Road, because of the uncleanly manner in which they had been kept. My complaint against these premises having been considered by the Licensing Committee of the London County Council, that body decided to withdraw the licenses. These places are now closed. LICENSING OF SLAUGHTER AND COW-HOUSES IN ISLINGTON, 1894. At the annual licensing in October, 1894, there were 61 applications for the renewal of licenses to Slaughter-houses, 59 of which were granted, and 2 were refused upon objections raised by the Vestry. For the renewal of licenses to Cow-houses 27 applications were made, and 25 were granted, whilst 2 were refused. 80 The annual inspection of these places, prior to the licensing, proved them to be generally in a satisfactory condition. Some defects were, however, noticed, which were considered of such a nature as to justify me in recommending the Vestry to oppose the renewal of the licenses. BAKEHOUSES. The Bakehouses were visited on several occasions during the year; no less than 441 visits having been paid. When their situations, mostly underground, and the difficulties encountered in ventilating them are considered, their state may be described as fairly good. As far as possible, every one was put into a cleanly state during the year. For my own part, I do not consider that it is wise to allow human beings to continue to labour in such places, for I think that the results to them are very serious. The only ground that has ever been raised against their abolition is that it would be too great an interference with the rights of property to cause them to be discontinued. It has always seemed to me a very absurd argument that because a place has been allowed to be occupied for a long series of years to the detriment of the health of the people working therein, that, therefore, it must not be now abolished. If these insanitary places have been occupied for such a long time, surely they have more than recouped their owners for the money that had been originally spent on their erection. To use a common phrase, "they owe nothing" to their owners. This being so, it is not too much to ask that in the interest of bakers generally, these places should be done away with. I hope, and believe, that the day is not far distant when every bakehouse will be situated above ground, where it can be freely and fully ventilated, and where the light of day can penetrate to every part of it and thereby create those healthy conditions which cannot possibly. be obtained in underground bakehouses. 81 Table LXIII.
